If you would like to enroll your child, please click the links below to print each form. You may mail the forms or bring them to the school and turn them in. Please first call Kingdom Kids to ensure there is an opening for your child.

Intent forms are due upon enrollment. All other forms are due no later

than the first Monday in August.

If enrolling after August, all forms are due upon enrollment.
